I have read and understand the clubs Policy on Insurance as follows:

PERSONAL ACCIDENT INSURANCE COVER PROVIDED BY MUAFC

Death Benefit €130,000
Loss of limb / Loss of eye €130,000
Permanent Total Disablement €130,000
Temporary Total Disablement (* see note 1) Up to €600 per week
Medical Expenses (* see note 2) €5,000

NOTES – Please read carefully

1. Temporary Total Disablement only applies to people in full-time gainful employment and only covers loss of actual earnings. If a player makes a claim the Insurance Company will deduct any payments made by the employer and Social Welfare/Revenue and will pay the balance to bring the player up to his/her average weekly wage. Average weekly wage will be defined as the net wage for the six weeks prior to the date of accident. Players must be able to provide proof of employment/earnings.

There is an excess period of 4 weeks before payment will be made. This means that the player will not receive any payment for the first 4 weeks immediately following the accident. Temporary Total Disablement is payable for a maximum of 26 weeks after the excess period. The excess period starts from the date of the accident.

2. Medical Expenses are subject to an excess of €100 (i.e. the player must pay the first €100 of all medical expenses). There is a limit of €250 for physiotherapy per player per year. If a player holds VHI, BUPA or similar health insurance they must claim under their own policy first. They can then submit a claim under club
insurance for any expenses not recovered under their own private health cover.

3. All claims must be notified to the administration office within one week of the accident. Failure to notify the club may result in claim not being paid.

4. Emergency Dental Cover only is provided , i.e only dental work carried out within the first 48 hours of the accident. You should check to ensure that your own private health cover provides adequate dental cover.

N.B. YOU ARE STRONGLY ADVISED TO TAKE OUT YOUR OWN PERSONAL ACCIDENT INSURANCE AS THE CLUBS COVER MAY NOT MEET YOUR FULL REQUIREMENTS IN TERMS OF MEDICAL COVER OR LOSS OF EARNINGS.
